Care Coordination & Administration
Strategic management of care plans, documentation support, and administrative advocacy for aging adults.
Referrals & Resource Advocacy
Facilitating communication with providers and connecting clients to essential community and professional resources.
Companionship & Transportation Coordination
Arranging transport for medical appointments, errands, and providing companionship for social wellness.
Client Responsibilities
Clients or representatives must maintain open documentation and provide accurate history for effective care management.
Cancellation Policy
To terminate a recurring monthly care coordination package, the Client must submit a formal written notice via email. Notification of cancellation must be received by Compassionate Partners LLC no less than fourteen (14) days prior to the subsequent billing date to prevent further recurring charges.
Fees for services already rendered are strictly non-refundable. Once a monthly service term has officially commenced, the package fee for that term is earned and non-refundable, except where required by applicable law or if the service is discontinued by the agency without cause.
Compassionate Partners LLC reserves the right to suspend or terminate service packages immediately for reasons including non-payment, inappropriate conduct toward staff, or safety concerns. In such events, fees for the remaining term may be forfeited at the agency's discretion.
Modification Policy: Any requests for changes to scheduled sessions or administrative support services should be communicated to your coordinator at least twenty-four (24) hours in advance of the scheduled service time.
Billing Terms & Methods
All service coordination fees for Compassionate Partners LLC are billed monthly in advance in U.S. Dollars (USD). We utilize a secure online payment portal that accepts major credit and debit cards. By establishing a service plan, you authorize recurring monthly billing to your designated payment method. It is the client's responsibility to maintain updated and valid payment information to avoid service interruptions.
Partial-month service starts may be prorated based on the initial activation date or effectively scheduled for the subsequent full billing cycle. Fees collected cover non-medical administrative support, companionship, and coordination efforts as specified in the selected package tier.
Refund Policy & Limitations
- Fees are strictly non-refundable once a monthly term has begun, as services are rendered on a best-efforts basis.
- Refunds for partial usage of a monthly period are not provided, except where required by law.
- In the event Compassionate Partners LLC cancels services without cause, a prorated refund will be issued for pre-paid days remaining in the term.
- Service coordination relies on client cooperation and accuracy of information; refunds are not issued for delays caused by third-party providers.
Important Notice: Declined or late payments may result in immediate suspension of all services, including communication with healthcare providers and family advocacy. Late fees may be applied to the extent permitted by law.
